Objective:To determine if the effects of orlistat (Xenical) are sustained by the use of orlistat or Xenical and to assess the impact of co-administered orlistat or Xenical and dietary fat intake on weight loss over time. Methods: This was a double-blind, placebo-controlled, single-case, crossover study in healthy volunteers with a diet- and/or exercise-induced, non-smoking, nocturnal insulin resistance. Subjects were required to adhere to the diet for at least 4 weeks before the study and were allowed to consume a diet containing either orlistat or Xenical for 4 weeks. Subjects were required to maintain their diet for at least 5 days before they began the study.
Methods:Participants were randomly assigned to a diet and/or exercise-induced, non-smoking, nocturnal insulin resistance (NIDEL) trial or a nocturnal insulin resistance placebo-controlled crossover study. After 4 weeks of NIDEL, subjects were switched to a diet and/or exercise-induced, non-smoking, nocturnal insulin resistance (NIDEL) study. The study was conducted in accordance with Good Manufacturing Practice and the requirements of the Declaration of Helsinki and Good Clinical Practice guidelines.
Results:The incidence of clinically significant clinically significant weight loss was significantly greater in orlistat-treated subjects compared to placebo-treated subjects. The mean change in BMI was −0.06, −0.04, and −0.01, respectively. There was no significant difference in waist circumference between the groups. Conclusions:The orlistat/Xenical trial is well tolerated in healthy adult male subjects with clinically significant obesity and a normal-weight, healthy, and stable metabolic profile. Further, the orlistat-treated subjects with clinically significant obesity and a normal-weight, healthy metabolic profile may have a greater risk of adverse cardiovascular outcomes than the orlistat-treated subjects without clinically significant obesity.
